HIV Nursing Case Management

The Southern Nevada Health District HIV Nursing Case Management Program provides services to meet the special needs and comprehensive care for HIV/AIDS impacted individuals.

The Nurse Case Manager promotes a systematic patient care delivery system that focuses on the achievement of physiological outcomes while utilizing community resources. Case management intervention is to assure that clients with HIV/AIDS receive coordinated care throughout their disease process.

Patients are accepted into the program only when referred by a doctor, who will be an active participant in the case management process.

Case Management services are provided free of charge. Funding for this program comes from Ryan Whilte Title 1.
